Transition Family Drop In Session

EACH are excited to be working with our adult colleagues across Norfolk and Waveney to bring children’s services together to develop our transition into adult hospices.
We’ll be holding monthly drop in sessions on Saturday’s to offer young people and their families an opportunity to meet our adult colleagues. They may be involved in your care when transitioning from EACH to adult services, you will be supported by the familiar faces of our EACH staff during this time.
Sessions will run from 2-4pm and there will be activities for young people and their siblings to participate in. It’s an opportunity for you to connect with other young people and families who are approaching transition.
We have sessions on:
- 28/09/24 – Louise Hamilton Centre, James Paget University Hospital, Lowestoft Road, Gorleston-on-Sea, NR31 6LA
- 19/10/24 – The Norfolk Hospice Tapping House, Wheatfields, Hillington, King’s Lynn, PE31 6BH
- 30/11/24 – Louise Hamilton Centre, James Paget University Hospital, Lowestoft Road, Gorleston-on-Sea, NR31 6LA
- 25/01/25 – Priscilla Bacon Lodge, Century Place, Colney, Norwich, NR4 7YA
- 22/02/25 – The Norfolk Hospice Tapping House, Wheatfields, Hillington, King’s Lynn, PE31 6BH
- 29/03/25 – Priscilla Bacon Lodge, Century Place, Colney, Norwich, NR4 7YA
These sessions are currently running as a pilot for 12-18 months but we hope we can develop further transition support/services across Norfolk and Waveney.
The drop in is an opportunity for young people and their families to help co-produce what these services may look like in the future, we want to listen to you and how we can support you through this journey. There will be the opportunity to input into this through a range of mediums.
Please note these are self-care sessions so you will be responsible for your young person and any other family members attending.
