A WEEKEND EVENT FOR THOSE AGED 12-17 WITH SMA _____________________________________________________________________________

*VERY IMPORTANT - PLEASE NOTE* 
Each booking consists of two tickets, one ticket is allocated for a child aged 12-17 with SMA, the other ticket is for a parent / guardian.

To take part in this unique, one-off event you will need to pay a £25 deposit for each guest.  This will then be refunded back to you after you have attended the event.

As soon as you have paid your deposit, you will receive your booking confirmation via email.  SMA UK will then be in contact with via email you to check your details and to ask you to provide us with some additional information (medical info, in-room equipment requirements, other specialist equipment requests, dietary info and emergency contact details).
