A WEEKEND EVENT FOR ADULTS WITH SMA

*VERY IMPORTANT - PLEASE NOTE* 
You can only order a maximum of 3 tickets per order.  1 ticket is allocated for an adult with SMA, the other ticket(s) are for your carer/pa(s).  If a member of your family is accompanying you as your carer/pa, please just add their details on the registration form as your carer/pa.

If you do not require the support of 2 carer's/pa's for this event, please only select a maximum of 2 tickets for your order.  By making sure you only select the number of tickets you actualy need, you will be freeing up more tickets and giving more adults within the SMA communtiy a chance to attend this event. 

To take part in this unique, annual event you will need to pay a £25 deposit for each guest.  This will then be refunded back to you after you have attended the event.

As soon as you have paid your deposit, you will receive your booking confirmation via email.  SMA UK will then be in contact with via email you to check your details and to ask you to provide us with some additional information (medical info, in-room equipment requirements, other specialist equipment requests, dietary info and emergency contact details).
