IMPORTANT INFORMATION:
Audit and Evaluation
As part of the Kick Start Taster Sessions you may be asked to:  
1. Complete an evaluation questionnaire at the end of the programme. 
2. Agree to being contacted at a future point in time for further evaluation & information. 
The details of the above will be recorded in a database for future audit, quality assurance and evaluation of the programme. Your name or other information that might identify you will not be sent to anyone but will be stored on a database. Please note that your confidentially is assured, it will not be possible to identify any individuals in the data base or in any research or evaluation reports.
Throughout the programme we will be discussing Physical Activity and its benefits.  Please check with your GP before undertaking any new exercise. 
I hereby consent to have my personal information in an anonymous way for example, my evaluation and attendance, to be used in the research, quality assurance and evaluation of the programme. 
I have been informed that it will not be possible to identify me or any participant in any database, research or evaluation reports. 
I am free to refuse to take part in the project and free to refuse to allow my personal information in an anonymous way to be used.
